EXTENSION OF REPORT FOR IDENTITY THEFT AND FRAUD
To Whom It May Concern / Houston Police Department (HPD):
By means of this document, I formally request an extension/supplement to the original incident report No. 1315077-22 (dated July 10, 2022), which was handled at the time by Officer B. Juanez (Badge No. 38530).
I officially state and reiterate the following facts so that they remain duly registered in the case file:
- The Initial Theft: Between May and June 2022, I was the victim of the theft of a briefcase that was inside my vehicle (a 2005 Ford Focus) parked near 5715 Airline Dr., Houston, TX 77076 (in the area of Taqueria Meat Market).
- Compromised Documents and Accounts: In said theft, my official identity documents (passport, voter ID card / consular registration) were stolen, as well as credit cards from Discover Bank and Credit One Bank (the latter being the institution linked to the current lawsuit by Resurgent Receivables LLC).
- Fraudulent Use and Duplicity: The stolen data was used illicitly to open or modify accounts in my name, altering information and using an address other than my home address (requesting a new card sent to Arturo Sanchez, 10002 Blissfull Valley Ln, Tomball, TX 77375; confusion with a homonym's address at [33, XX] Parker St., TX 77076; while ignoring my real address at 5412 Nordling Rd., 77076).
- Purpose of This Extension: I am presenting this detailed breakdown in compliance with the procedural request required by the opposing party and the judge in the ongoing civil case, demanding that the original identity fraud be formally recognized.
Final Request: I request that this extension be integrated into the original police report file in order to proceed with the formal cancellation of the fraudulent charges associated with these accounts, and that the necessary steps be taken to clear my history with the credit bureaus, exempting me from any responsibility for debts contracted by third parties through the theft of my personal documents.
AMPLIACIÓN DE DENUNCIA POR ROBO DE IDENTIDAD Y FRAUDE

Por medio del presente escrito, solicito formalmente la ampliación del reporte original de incidente No. 1315077-22 (con fecha del 10 de julio de 2022), atendido en su momento por el oficial B. Juanez (Unidad 38530).
Hago constar y reitero de manera oficial los siguientes hechos para que queden debidamente registrados en el expediente:
• El Robo Inicial: Entre los meses de mayo y junio de 2022, fui víctima del robo de un maletín que se encontraba dentro de mi vehículo (un Ford Focus 2005) estacionado en las inmediaciones de 5715 Airline Dr., Houston, TX 77076 (zona Taqueria Meat Market).
• Documentos y Cuenta Comprometidas: En dicho robo sustrajeron mis documentos de identidad oficiales (pasaporte, tarjeta de elector / matrícula), así como tarjetas de crédito bancarias de las instituciones Discover Bank y Credit One Bank (la misma institución vinculada a la actual demanda de Resurgent Receivables LLC).
• Uso Fraudulento y Duplicidad: Los datos sustraídos fueron utilizados de manera ilícita para abrir o modificar cuentas a mi nombre, alterando información y utilizando Direccion ajena a mi domicilio (Solicitando nueva tarjeta enviada a.
